The Ipswich and East Suffolk Alliance Awards Evening

Over 100 guests attended the inaugural Ipswich and East Suffolk Alliance Awards Evening at Endeavour House on 12 December 2023.

The evening was dedicated to recognising how individuals, teams and projects from across the Alliance have excelled in demonstrating the Alliance’s values.

Many thanks to everyone involved, and to everyone working hard to continually support our communities. Browse a list of the awards, winners and comments from the panel below.

Overall Winner – Woodbridge Holistic Assessment Clinic Team

This team were judged by the panel to have demonstrated all six of the Alliance’s values.

They have embraced the ‘One Team’ development programme and used population health management along with their professional insights to create something quite new – a holistic service for older people in their community who had had a fall or were at risk of falling.

They brought together matrons, social prescribers, podiatrists, pharmacists, therapists and social workers to enable people to come with their carers for a single assessment of their needs and forward personalised care plan.

They have positively embraced the support of their transformation manager, who they describe as “beyond organised, kind and clever”.

Their approach has saved patients from multiple journeys and they have managed to create and run this new service using their existing resources and small pots of money for hall hire and screens.

Their innovative work has also been shortlisted for the HSJ partnership award.

Congratulations to Nikki Pemberton, John Lynch, Ruth Mills, Karen French, Heather Thompson, Ginette Smith and Sophie O’Leary, as well as their transformation manager, Angela Ashton.


Silver Award – The Connect Team

Congratulations to Chris Lawson, Stuart Hasley and Michael Robinson who have brought together our voluntary, community and wider statutory partners in each of our Integrated Neighbourhood Team (INT) localities – listening and learning from our communities, giving them a voice and aligning funding to make change happen.

Their nomination cited them as ‘awesome ambassadors for our values and behaviours’.

Special mentions went to:

Andy Wrench – a phenomenal listener who keeps us all on track and in touch through the newsletters.

Antonia Lancaster – a proactive positive project lead

Emma Doughty, Quentin Cass and Anson Lockwood– for making co-location for our teams a reality.

Public Health Team – for giving us the information and the skills to use it to make our INT and Connect plans really focused on the needs of people who need us most.


Silver Award – the Shaw Trust Connect for Health Social Prescribing Team

The panel recognised the unique contribution of the Connect for Health Shaw Trust in all six categories.

To cite just two categories, the panel were impressed by their compassionate, personalised care for individuals in six Integrated Neighbourhood Team areas and their collaboration with statutory and voluntary organisations to ensure that their needs are met.

The panel also made a special mention of Hayley Stearn of Shaw Trust, who was personally nominated for proactive and sustained leadership of the programme, and Nicole Rickard from East Suffolk Council for stepping forward with courage to embrace a whole new partnership arrangement with the Alliance to commission social prescribing on our behalf.

Winner – Transfer of Care Hub, Ipswich Hospital

For collaboration with patients, families, care homes, transport providers, hospital teams and social care to help people get home .

Award received by Vicky Bazely, Hannah Beardmore and Andrea Barfield.


Winner – Stepping Home

For supporting our most vulnerable patients with a highly personalised service to manage all of the complexities that they may face with housing and social needs.

Award received by Gary Crockett, Teresa Howarth and Heather Tucker.


Silver Award – Very Important Invitation from Suffolk GP Federation

For addressing inequalities in cervical screening by working in partnership with GP practices, hospitals, charities and most importantly, our communities to offer extra and highly supportive appointments in the evenings, at weekends and bank holidays. This is really helping to meet the goal of diagnosing 75% of cancers at stage one or two by 2028.

Award received by Lucy Ainsley, Maddy Dunningham, Hannah Ismay and Ashleigh Townes.


Silver Award – Hadleigh Health Centre’s Care Home Team

For Hadleigh’s care home team, which includes a PCN Clinical Pharmacist, PCN Care Coordinator, Lead GP and Lead Nurse, demonstrating a ‘one team approach’ with their care homes to provide an enhanced package of care which supports patients and the care home teams.

Award received by Dr Gillian Croot, Kate Brett and Julie Mills.

Winner – Live Well Hub Leiston

For the creation of a truly community focused offer, originally conceived by Connect group members and taken forward by the Integrated Neighbourhood Team, then joined by partners from across health, social care and local government, specifically including the Town and District Councils.

The hub provides a single location where the local community can access a range of services to improve their physical and mental health and wellbeing.

Silver Award – ESNEFT, East Suffolk Community CDG Management Team

For a Team of exceptional leaders that support all the specialist services, REACT and INT teams in east Suffolk working in our communities to deliver care to people in their own homes. During Storm Babet they ensured their teams were safe and that care was delivered safely by organising the 4×4 service.

Silver Award – Felixstowe Integrated Neighbourhood Team & Felixstowe Minor Injuries Clinic

For absolute dedication to the people of Felixstowe. This team, led by Evelyn Catto, is continuously adapting to changing circumstances but with just one goal in mind – to join up care and make the lives of people in Felixstowe better.

Since the closure of the walk-in clinic at the hospital they have continued to provide minor injury, phlebotomy, leg ulcer management and wound care and have now added hypertension reviews, initial diabetes assessments and ECGs, support for Parkinson’s Disease and Frailty assessment clinics.

They also think about how the little things in life that make a big difference such as the availability of hearing aid batteries for collection from reception. They are proud – as are we – of their ‘can do’ attitude to help the people of Felixstowe.

Silver Award – SPOT Wellbeing

For a true focus on individuals wellbeing within their community – giving people time to really understand what matters to them and provide holistic support to retain independence, improve confidence and wellbeing and reduce social isolation.

Award received by Holly Simpson, John Ottaway and Lynn Sibley.


Silver Award – Outdoor Wellbeing Experience Programme, Food Museum Stowmarket

For building a community from a life changing programme. The Food Museum model of volunteering is offering sustainable year-round support for people in Stowmarket and the surrounding area. Whilst the core programme runs 10 weeks, in reality, everyone is welcomed to remain part of the volunteer team at the Museum and can even become mentors for the next intake.

Silver Award – Suffolk Artlink

For developing and delivering a programme of creative arts to people who need us most and for growing a community of practice by working in partnerships with our local libraries.

Award received by Alex Casey and Hayley Field.

Winner – Sarah and Dennis Tattoo, St Elizabeth Hospice

For their compassionate, kind and utter dedication to people who have suffered bereavement – Sarah and Dennis began 4 mile and 8 mile walks with a café stop so like-minded people who have suffered loss can support one another at a very difficult time in their lives.

Over two years they have grown the group to over 50 people who have developed lifelong friendships. Their nomination said that ‘Nothing is ever too much trouble for them, and they deserve recognition for all that they do’. The Panel couldn’t agree more.

Silver Award – Dr Nicola Maggs, Leiston Surgery

For Dr Maggs, whose nomination described her compassion as rooted in ‘taking the time to listen’, to understand a patient’s needs and wishes and do everything she can to help deliver them, for each of her patients, in their time of need.

Silver Award – Cohere Arts

For the amazing kind and creative arts Cafe sessions in Holywells Park in Ipswich, which enables people to join a community of kindness, reduce loneliness and without maybe even knowing initially, to heal.

A recent participant recently said; “This is changing my life.”

Silver Award – Green Light Trust

For the Green Light Trust, offering green social prescribing interventions as part of the complex patient pathway whose offer in a wonderful woodland setting enables people to recover and to ‘be well’ at their pace. From lying in a hammock to wood carving and cooking over the open fire – this is totally led by each participant, and this is truly personalised.

Special Mention – Complex Patient Team

We would like to give a special shout out to the Complex Patient Team within the ICB – to Jane, Jeni and Shelley for working in total partnership with our VCSFE to create the most personalised and compassionate programmes whose work has been awarded tonight – Cohere Arts, the Green Light Trust, Suffolk Art Link and the Food Museum.

What you do may not always been seen or even understood but it is leading change for individuals and our system.

Winner – Cardiology Advice and Refer Pathway Team

For a partnership between secondary care and primary care with patients at it’s centre (you may say at it’s heart!), enabling:

  • 80% of referrals being offered rapid specialist advice within 48 hours
  • 70% of patients to be managed in primary care, closer to home
  • A 30% reduction in cardiology outpatient appointments, saving nearly £90,000 each year increased consultant productivity with six patients rather than two being seen per hour
  • a saving of 11,500 patient travel miles are being saved per year helping to reduce carbon emissions

Silver Award – Gemma Hearsum, Two Rivers and Felixstowe Road PCN

For Gemma, within her pharmacy technician role at Two Rivers and Felixstowe Road surgeries, who always goes above and beyond to help support the work in practice. She has driven forward work with the practices and Alliance Medicines Management team to help patients make the most of their medicines and manage the money.

Winner – ReSPECT Project

For the vision, tenacity and courage to roll out ReSPECT, enabling a Patient Centred – shared decision-making record about end of life care wishes and continuity of care between professionals such as ambulance crews, out-of-hours doctors, nursing & residential, hospice and hospital staff to make immediate informed decisions about a person’s emergency care and treatment.

Silver Award – Central Suffolk Integrated Neighbourhood Team

For driving forward collaboration between the community, social care, mental health, social prescribing and primary care partnership to deliver a collective ‘on the day’ response to people’s needs and proactive approach to people with the highest complexity.

Michelle has shown courage, compassion and collaboration. She has faced some difficult times but has led her team (health and care professionals) through complex change always keeping the individual (patient, carer, their family and her staff) at the heart of what she is doing.

Silver Award – Phanuel Mutumburi

For Phanuel – with these exact words from his nomination… ‘The very nice way of challenging us to ensure we always consider and think of equity, equality and inclusion. His extremely professional and mindful way leaves a lasting impression, one that educates us, develops us, both personally but as an Alliance that believes in its values.’

Winner – The Unity Centre

With special acknowledgement of Louise Hardwick and Dean Dorsett.

For Louise and Dean who understood – through listening and learning with the community – the need and opportunity to develop a new approach to meeting health needs, starting in Ipswich West at the Unity Centre. And to use that learning to begin the process of sustaining and scaling locally developed Hubs in all of our Integrated Neighbourhood Team and Connect areas.

Silver Award – Proxy Access for Care Homes Project Team

With special acknowledgement of Julia Smith, Hannah Wattam and Sarra Bargent and continuously supported by Jen Burman and Scott Pomroy.

For improving the lives of our patients in care homes by enabling access to on-line medication ordering; meeting local needs and far exceeding national targets within an overall programme of support for care homes and practices.

Silver Award – Suffolk GP Federation Podiatry Team

For creating a whole new approach to managing foot services through involvement and education; improving access; reducing ulcers and amputations.
