When a patient has an unexpected stay at a hospital, their home environment may be unsuitable for a safe discharge. The Hospital Discharge Service team can help make their home environment safer, from rearranging furniture to accommodate medical equipment to ensuring kitchens and bathrooms are clean and as easy to navigate as possible.
