On this page you will find information about how the ICB uses your information. This information is a shorter version of our full privacy notice.
Who are we
We are the NHS Suffolk and North East Essex Integrated Care Board (ICB).
What do we do?
Our job is to plan and arrange health services near where you live. This includes the doctors and the hospitals. To do this we sometimes need some of your personal information.
What is personal information?
Personal information is information about you. It might be your name, your address or information about your health. If you’re poorly, we might have information about that.
How long we keep your information for?
We keep your information for as long as we are allowed to and then we delete it.
Why do we have this information?
We have this information for a few reasons:
- To help us plan health services you might need.
- To keep you and others safe.
- To follow the law
We might collect this information from you, your family and other people involved in giving you this help.
Giving your permission
Sometimes we will ask your permission to use your information.
When we ask for your permission this is called consent.
You do not have to give consent if you don’t want to though. This is your choice.
If you are under 13 you may not be able to give consent on your own and we will ask your parent or guardian.
We may need to share your information with other organisations that care for you, to help keep you safe.
What decisions can you make about your information?
You can ask for your information to be corrected if it is wrong.
You can ask what information we hold about you and we can give you a copy of it.
Who can I contact?
If you have any questions, you can email Paul Cook. Paul is called our Data Protection Officer, and he looks after all our information.
The email address is dpo@snee.nhs.uk
